Oncology Patient Navigator

Jefferson Healthcare is seeking a full-time Oncology Patient Navigator to join our Oncology Center team. Patient navigation in cancer care involves providing personalized assistance to patients, families, and caregivers to help them overcome barriers to care and ensure timely access to quality medical and psychosocial support from diagnosis through all stages of the cancer journey. The Oncology Patient Navigator manages resources through interdisciplinary collaboration to achieve the best possible care outcomes for oncology patients.

What you'll need:

• High school diploma or GED required
• One (1) year of customer service experience required
• Associate's Degree (in a major like Social Work, Sociology, Education, or Psychology) preferred
• Experience in a health care setting preferred
• Previous work experience in Oncology Clinic setting is ideal

Schedule: Full-Time; Day shift (0730-1600)
