Jefferson Healthcare, a leading and award-winning healthcare system, is seeking a full-time Nurse Practitioner or Physician Assistant to join our Palliative Care Team. This position primarily involves providing home-based care, with some clinic visits and occasional inpatient consultations. It offers a unique opportunity to make a meaningful impact in a close-knit rural community. We're looking for a compassionate and experienced provider who excels both independently and as part of a team, with expertise in Palliative Care, Home Health, and Hospice services.

The ideal candidate will be Board Certified/Board Eligible, have experience in both inpatient and outpatient settings, should be comfortable with geriatrics and a wide range of clinical conditions, and be motivated to live in a rural area while delivering patient-centered care.
